Loan Request Form

• Orders and requests will be shipped by UPS or U.S. Mail. Please supply both mailing and street address and allow 14 days for delivery.
• Payment is required in advance of shipment! Personal checks and credit cards accepted. Make checks payable to: Mackinac State Historic Parks
  
• For Loan Materials: User is responsible for return shipping.
  
• An invoice can be issued for Education Materials only if requested on school letterhead or accompanied by a Purchase Order. Please attach our order form.
  
• During peak request times, orders for Loan Materials will be shipped on a first-come/first-served basis.
